Ideally, these conversations begin before a diagnosis or hospice care, or as soon as you or a loved one receives a diagnosis or signs on to hospice.

During our sessions, I guide you with thoughtful questions to uncover what matters most to you. I take careful notes, organize your responses, and provide a clear written summary of your decisions. You’ll receive a printed or digital PDF copy you can share with loved ones or medical providers, ensuring your wishes are honored when it matters most.
Our End-of-Life Care Plan goes beyond standard forms. It addresses emotional, spiritual, and practical needs, guiding you through choices that matter deeply. You’ll also find tools to help you talk with loved ones and make decisions that reflect your values and vision for the end of your life.
